How to Donate
All states—including Wisconsin—have enacted the Uniform Anatomical Gift Act (UAGA) which allows all people age 18 and older to donate eye tissue after death for transplantation, research, and education.
There are several ways that you can ensure your future donation for transplants:
- Sign an Eye Tissue Donor Card that indicates your wishes regarding donation
- Place your donor dot on your drivers license, and sign it.
- Talk to your family and friends about donation. Let them know of your wishes.
Even in the presence of a signed driver's license with a donor dot, Lions Eye Bank of Wisconsin will act only with the consent of the donor’s legal
- Spouse
- Adult son or daughter
- Either parent
- Adult brother or sister
- Either grandparent
- Legal guardian
- Healthcare power of attorney
